Assessment will measure the person's ability to perform the basic activities of daily living such as bathing, dressing, eating and mobility.

Care Advisory Services help to assess and care planning by a Home Health Agency, a Care Management Organization or Independent Care Manager.

Care Management Services an organization which is licensed to provide Care Advisory Services according to the laws of their jurisdiction and has a full-time administrator. They must maintains records of services provided to each client. Their staff must include a registered nurse, licensed social worker, an individual that holds a "Care Manager" designation or has a Masters in Gerontology.

Care Manager is a person who is usually a registered nurse or licensed social worker that assesses the needs of the patient. They coordinate the delivery of long-term care and services which they monitor throughout your period of care.

Licensed Health Care Practitioner is a Physician, a registered nurse, a licensed social worker, or any other individual who meets the requirements of licensing prescribed by the State in which care is provided.

Plan of Care means a written plan for long-term care services designed especially for you. This Plan of Care must specify the type, frequency and providers of all the services you require; and be in accordance with accepted medical and nursing standards of practice. A Licensed Health Care Practitioner must approve Your Plan of Care.
